A decade after having undergone placement of a right hip prosthesis, a 73-year-old man with type 2 diabetes mel-litus presented with a 3-month history of worsening right hip pain together with a right lateral thigh mass. The patient, who denied having had fever, resided on a farm with animals, including sheep, and had also extensively traveled in his work as an agriculture consultant. C-reactive protein (CRP) and erythrocyte sedimentation rate (ESR) were normal. He had a 15cm×9cm tender right lateral thigh mass and magnetic resonance imaging revealed subcutaneous fluid with collections around the acetabulum. The nucleated cell count of aspirated periprosthetic fluid was 5750 cell/mm~3 and 88% were neutro-phils. Fluid cultures (bacteria, mycobacte-rial, fungal) were negative.
