Foundation: New York State Health Foundation. Innovation: Five-year, $35 million campaign to reverse the diabetes epidemic in New York State by improving clinical care, sustaining a comprehensive and coordinated care system, and promoting prevention. Challenge: Nearly 1.8 million New Yorkers-almost 10 percent of the state's population-had been diagnosed with diabetes as of 2008. Commitment: Help 3,000 primary care physicians and other providers meet diabetes care recognition standards set by the National Committee for Quality Assurance or Bridges to Excellence. Assist small primary care practices, hospital outpatient departments, and community health centers in transitioning to the Chronic Care model. Results: So far about 1,000 providers in the state have met the target standards and achieved diabetes care recognition. It's too early to measure other results, such as reduced hospitalization. Next Steps: The foundation is now awarding grants to additional organizations to help more physicians meet diabetes standards, adopt the Chronic Care Model, and improve their care for a wider range of chronically ill patients. There is broad recognition that payment reform and pay-for-performance incentives will be needed from the majority of payers in the state to achieve superior results on costs and quality improvement.
