Hyperkinetic catatonia is a rare syn-drome found in mood and psychotic disorders characterized by features such as purposeless motor hyperactiv-ity, extreme agitation, and echophe-nomena. Although general catatonia is often treated with benzodiazepines and electroconvulsive therapy (ECT), there is no standardized treatment for hyperkinetic catatonia. The case presented here is that of a 44-year -old woman with a diagnosis of schizo-phrenia with hyperkinetic catatonia. Included is a review of the clinical evaluation and diagnostic impres-sions, as well as approaches to man-agement and treatment. The patient underwent multiple sessions of ECT that yielded significant improvement in disorganized behavior and hyperac-tivity. This case demonstrates the di-agnostic challenges in differentiating hyperkinetic catatonia from delirious mania and other mood disorders, and the effectiveness and safety of ECT in a patient with hyperkinetic catatonia.
