The rotator interval (RI) has recently become a popular topic of discussion and debate. The anatomy and structure of this area of the glenohumeral joint has been well defined. Its contribution to the stability of the glenohumeral joint has also been investigated, and many bio-mechanical studies have demonstrated the important role the RI plays in providing inferior and posterior stability to the glenohumeral joint. Several publications support the use of RI plication in the setting of glenohumeral instability. This article reviews the anatomy and function of the RI. The biomechanical and cadaveric studies that provide validity for RI surgery are also summarized. Finally, our technique for arthroscopic RI plication is presented.
