Tenodesis of the Palmaris Longus Inside the Clavicle in Order to Treat Painful Recurrent Sternoclavicular Dislocation

A recurrent Sternoclavicular dislocation is a rare affection. Many surgical procedures have been described to treat this instability. In the rare cases where painful osteoarthritis of the joint is associated, a resection of the internal third of the clavicle has to be performed. In order to stabilize the clavicle, we describe a procedure using the palmaris longus tendon harvested on the same side upper-extremity and fixed inside the resection of the clavicle by a tenodesis screw. The tendon is passed twice time around the first rib, just under the end of the clavicle resection. Postoperative chest anteroposterior x-ray has to be performed to identify possible pneumothorax. In our experience, there is no recurrence of pain or impairment of upper extremity movement, but recovery takes 3 months to allow total mobility.
