Clinical effectiveness of topical negative pressure for dermal substitutes in burns

The article by Bloemen et al. in this issue suggests that dermal substitutes can be covered immediately with thin split-thickness skin grafts if they are covered and treated with topical negative pressure. This editorial provides a different view when it comes to the need for using topical negative pressure and raises concerns as to whether dermal substitutes should be immediately covered. I would like to commend Dr. Middelkoop and her team for performing a randomized controlled trial to test two hot topics in burns-whether skin grafts can be placed immediately on a dermal substitute and whether "topical negative pressure" makes a difference in graft take.
