The closing speed of two people walking isn't particularly high and, as a result, news reports of pedestrian pileups only exist in the minds of comedy writers. At the basic level people can generally see each other, even if it is at the last minute, and take negotiated avoiding action, which at worst will be a bit of sideways dancing. People on foot are not always predictable (because they are being people) and from a design point of view this can be terrifying for someone used to dealing with the movement of motor traffic, which is more traditionally about management and predictability (and even then, we forget that humans are doing the driving).
