Sepsis is defined as "a condition of life-threatening organ damage caused by infection". Because early treatment intervention is important for improving prognosis of sepsis, an establishment for early screening marker would be required. So far, white blood cell (WBC) count, C-reactive protein (CRP), procalcitonin (PCT), and presepsin (P-SEP) have been suggested as potentially useful biomarkers of sepsis. However, while these parameters are useful to predict inflammatory condition, they would not directly reflect the pathophysiological status of sepsis. Therefore, an exploration for the better biomarker of sepsis would be important. Monocyte distribution width (MDW), which reflects the amount of monocyte variation in volume and size, can be measured with automatic blood cell counter UniCel DxH 900 (Beckman Coulter Co., Ltd.). Because circulating neutrophils and monocytes have an important role to immediately eradicate pathogenic microorganisms, the detection of the changes in the volume of monocytes based on MDW has been suggested to be a promising early biomarker for sepsis. In this study, we evaluated the usefulness of MDW by comparing the association with other parameters (WBC, CRP, PCT) and SOFA (sequential organ failure assessment) score, based on clinical samples derived from septic and aseptic(non-sepsis) patients.
