Ultrasonography of Drop Finger: Nontraumatic Tear of the Extensor Indicis and Extensor Digitorum Tendons to the Index Finger

A 59-yr-old right-hand-dominant man with insulin-dependent diabetes mellitus, hypertension, and hyperlipi-demia presented for evaluation of an inability to extend his right index finger for several months. This was accompanied by mild swelling, intermittent pain, and numbness in the hand and the fingers. There was no preceding trauma. He denied other focal weakness in the right upper or lower extremities. No ulnar or radial deviation of the wrist was noted at rest. Strength of the right upper extremity was normal except for the inability to extend the second digit at the metacarpophalangeal joint. Flexion of the wrist was not accompanied by extension of the metacarpophalangeal joint of the index finger (extensor tenodesis effect). He was otherwise neurologically intact. The findings from laboratory work-up for inflammatory arthropathy and imaging (including magnetic resonance imaging of the brain) were unrevealing.
