Not so long ago, few questioned how the medical profession selected its aspirants, educated its students, trained its residents, credentialed its independent practitioners, and monitored their performance throughout their professional lifetimes. It was taken for granted that the various licensing and accrediting bodies, as stewards of the profession, knew what they were doing—an assumption seemingly warranted because the enterprise appeared to be functioning quite well. After all, everyone believed that the United States had the “best medical care in the world.”
