RISKS for patients when they are admitted to hospital or are moved, say, to a nursing home have been highlighted in a campaign spearheaded by the Royal Pharmaceutical Society - because information about their usual medication does not always travel with them. An average of 1.3 omitted drugs and incorrect doses were identified by staff in a recent evaluation of pharmacy-led medicines reconciliation within 30 NHS trusts (PJ, 9 July,p57).
