Editorial Comment: With the growing belief that Gleason grade 3 cancer may represent a nonlethal form of disease, there is increasing interest in finding ways to better stratify risk among men with Gleason score 7 prostate cancer. In this study the authors present a weighted scale for assessment of the relative contribution of Gleason grades 3 and 4 in men with known Gleason 7 disease. Interestingly this weighted, or quantitative, Gleason score is more predictive of disease outcome than the traditional Gleason score.
