These authors evaluated the association between DM and PCa in a large post-radiotherapy population at academic and nonacademic American centers. Although the results show that DM was associated with a higher risk of high grade PCa independent of race and similar associations were also found in postoperative (reference 16 in article) and biopsy1 series, we must confront them with previous large epidemiological studies in which an overall inverse relationship between DM and PCa was found (reference 13 in article). To our knowledge whether the conflicting evidence is due to dissimilar populations or to different study designs remains to be determined.
