This is a retrospective review of the radical cystec-tomy and urinary reconstruction experience at a leading urological oncology center in the United States. The study clearly shows decreasing use of continent urinary reconstruction from 2000 to 2005, which presumably also reflects current trends in use. However, because of the retrospective nature of this study and absent quality of life data, we can only speculate on the causes of this shift. Clearly advancing age, female gender, increasing medical comorbidity, locally advanced disease and surgeon preference are the major factors.
