Cataract surgical problem

This case urgently requires surgical repair because of the very low ECC. I see 2 critical aspects: (1) removal of the AC IOL, particularly of the incarcerated haptic loops, and (2) correction of the resulting aphakia. Under protection of OVDs, I would start with cutting off the haptics from the IOL optic. The next step is the gentle preparation of the haptics. If the loop ends are buried too firmly, I would cut them at the farthest point and leave the ends in the fibrotic tissue.
