The ISM code was introduced when statistics confirmed that around one third of the world fleet and owners/operators of ships were substandard. Shipping was the only form of transport remaining that did not require some form of licence to operate. So how did this happen? How did such a huge chunk of the world fleet become substandard and who is responsible for enforcing the rules and regulations in shipping?
