Didn't do the quality check, pressed the wrong button, put a hand inside the machine, forgot the instructions? How many times have you heard these something similar? Ask yourself what the total cost of accidents in your organisation is. Do you track it as a key performance measure? The chances are it is a substantial proportion of your total turnover. So what are you doing about it? You may have ISO 9001 or equivalent - and of course these standards will help to some extent. However, unless you also have a specific programme to tackle the behavioural causes of accidents and errors, you are missing the biggest opportunity. The word 'behaviour' is often used to cover unintentional human errors as well as more deliberate actions such as breaking the rules. It's not just about front line workers - the actions and behaviours of managers can also contribute to increased accident risks.
