Spontaneous Abscess of the Anterior Abdominal Wall Caused by Clostridium perfringens Resistant to Clindamycin

Spontaneous infections of soft tissues are rare entities that usually occur in immunosuppressed patients and are caused by Clostridium septicum in association with unknown gastrointestinal neoplasia or by Clostridium perfringens in association with previous surgery or penetrating skin trauma [1-6]. Both organisms usually are sensitive to clindamycin [2]. A 68-year-old patient arrived at our emergency department in poor general condition, having had abdominal pain for three days and a diminished level of consciousness. She had severe sepsis with tachycardia, tachypnea, high temperature, and arterial hypotension. Physical examination snowed a large, painful, indurated mass on the lower abdomen.
