A catalogue of errors led to 2010's devastating Deepwater Horizon oil spill in the Gulf of Mexico, which claimed the lives of 11 people and spewed out around 4.9M barrels of oil causing widespread environmental damage in the process. Perhaps most significant is the fact that the catastrophe might have been avoided entirely if rig owner Transocean had learned from an "eerily similar near miss" that occurred on one of its rigs in the North Sea in December 2009.
