Releasing its report into a boiler failure at the Kirklees Light Railway (KLR), on July 3 2011, when the driver did not notice that low water levels had caused the boiler's fusible plug to operate, the Railway Accident Investigation Board (RAIB) has raised its concerns about the increasing number of heritage railway incidents that have resulted from inadequate safety management systems (SMS). The RAIB has warned that heritage railways must get their house in order, or face a tougher operating regime - which could see their exemption to requiring full safety cases withdrawn.
