We would like to correct a typographical error in the description of the feed-forward transforms at the end of the thirdnparagraph in Sec. II C. The sentence “In this case, we flip the assigned values as described in the table so that j = 1 and k = 0nare obtained.” should be corrected to read “In this case, we flip the assigned k value as described in the table so that j = 0 andnk = 0 are obtained.”
