An initial report into a tower block fire in Cam-berwell, London, in which six people died and 15 residents and one firefighter were injured has highlighted a number of issues of concern.rnSir Ken Knight, the government's chief fire and rescue adviser, was asked to carry out an urgent review of circumstances surrounding the fire so that emerging lessons could be identified immediately to reduce the risk of similar fires.rnThe blaze, which happened in July, is believed to have been started by an electrical fault in a portable TV set which was in a flat on the ninth floor of the 14 storey block, but it soon spread to other flats and other floors, despite firefighters being on the scene within minutes. Discovering why the fire spread so quickly is a key focus of investigations.
