A Fatal Accident Inquiry (FAI) into the death of a 14-year-old army cadet who died while taking part in a training exercise on a Scottish loch has revealed that the cadets had been issued with the wrong lifejackets and that no detailed risk assessment had been carried out before the trip began. In August 2007, teenager Kaylee Mclntosh was taking part in a three-boat training exercise in the Outer Hebrides organised by the Army Cadet Force Association (ACFC), when the boat she and 11 other cadets were in capsized in choppy water. Kaylee became trapped underneath the upturned boat and drowned.
