The Eye of the needle report points out that where the HIV status of the source patient is unknown, the risk assessment of whether or not to continue on HIV antiretroviral drugs should take account of the risk profile of the patient: known drug use, male-to-male sex, country of origin etc. Surely, the act of risk profiling when it comes to post-exposure management is at odds with the concept of 'universal precautions'- ie that all patients are considered as potential sources of infection with a bloodborne virus - when it comes to prevention?
