Operations involving the assembly of nuclear warhead components at the Y-12 National Security Complex were temporarily suspended in August after an operator admitted using handwritten crib notes instead of the technical specification sheets for the high-security work. The incident raised questions about the conduct of operations at one of the nation's most sensitive nuclear installations, but National Nuclear Security Administration chief Thomas D'Agostino said the incident illustrated that the quality assurance program at Y-12 was working and he said the overall benefit was positive. An error occurred during the operation as a direct result of the worker using the cheat sheet, according to an Aug. 15 Defense Nuclear Facilities Safety Board report only recently made public after passing through a classification review.
