The nrc has issued entergy two "yellow" findings (findings of substantial safety significance), as well as a notice of violation, in connection with the accident at the company's Arkansas Nuclear One (ANO) plant in March 2013. The accident occurred when a temporary overhead crane that was being used to lift a generator stator during a refueling outage at Unit 1 collapsed, killing one worker and injuring eight others (NN, May 2013, p. 21). The accident also caused Unit 1 to lose off-site power, requiring Entergy to rely on emergency diesel generators to supply power to the reactor's cooling systems for six days. In addition, Unit 2 automatically shut down when vibrations from the impact of the 525-ton stator's hitting the turbine floor caused a reactor coolant pump to trip.
