The shockwaves of the Tokai-Mura accident continue to reverberate while those most exposed are in a serious condition. The health of the three workers severely irradiated in the Tokai-Mura accident is now the primary concern. The welfare of another 66 individuals exposed to significant doses is also vital. The cause was primarily human error and serious breaches in safety principles. These are the conclusions of a preliminary report by the International Atomic Energy Agency into the 30 September incident, which has been finally classified as level four on the International Nuclear Event Scale after provisionally being uprated to level five.
