ALGORITHME POUR LE TRAITEMENT SIMPLIFIÉ DE LA PYÉLONÉPHRITE AIGUË NON COMPLIQUÉE DANS UN SERVICE D'URGENCES

The treatment of acute pyelonephritis in the adult is controversial, although this infection is very frequent. Jraditional care consists of antibiotics parenterolly administered during hospitalization. It has been recently suggested that home care with oral monotherapy was safe for women with acute uncomplicated pyelonephritis (AUP). We have conducted, in me loriboisiere Emergency Department, a pilot study in order to identify a population of patients with AUP. In this case doctors were encouraged to discharge patients immediately or after a short observation period in the ED Observation Unit (EDOU), with oral pefloxacin. Of S3 patients treated for acute pyelonephritis, 79 were females, 65 of whom had AUP. Favorable clinical course at 3-week follow-up was observed in 98% of patients observed in the EDOU (n = 52) and in 92% of those immediately discharged (n = 13) (NS). We conclude that more than 80% of females treated in the Emergency Department for AUP may be discharged after careful evaluation of their clinkal status. Jhe EDOU was a key element in designing a home care strategy.%Malgré la grande fréquence de la pyèlonéphrite aiguë (PNA) de l'adulte, son traitement n'est pas codifié. Hospitalisation et antibiothérapie par voie parentérale sont habituelles. Il a été récemment suggéré que le traitement par monothérapie à domicile était sans risque dans le cas de la PNA non compliquée féminine. Une étude pilote dont le but était d'identifier une population répondant aux critères de PNA non compliquée, a été conduite aux Urgences de Lariboisière. Un algorithme décisionnel autorisait dans ce cas, le traitement ambulatoire d'emblée ou après quelques heures de surveillance dans l'Unité d'Hospitalisation aux Urgences (UHU). Des 83 malades traités pour PNA, 65 avaient une PNA non compliquée. La guérison a été obtenue pour 98% des malades observées à l'UHU (n = 52) et 92% de celles traitées d'emblée à domicile (n = 13) (NS). Nous concluons que plus de 80% des femmes consultant aux urgences pour PNA non compliquée pourraient être traitées à domicile. L'UHU est un élément clé de cette stratégie.
