STRATÉGIES THÉRAPEUTIQUES DANS L'ÉTAT DE MAL ÉPILEPTIQUE. LES URGENCES NEUROLOGIQUES : LES 60 PREMIÈRES MINUTES

The management of status epilepticus has improved over the past 20 years. Jhe outcome depends largely on aetiology, but prompt and appropriate pharmacological therapy can reduce the future morbidity and mortality. Risk of central nervous system injury increases significantly after 30′ of seizure activity and therefore our personal experience, as the most recent studies, stress a sixty minute time-window from the diagnosis of status epilepticus to its successful control. The key of such success is the knowledgeable and prompt use of high-dose intravenous Clonazepam, followed by Phenytoin. Refractory status epilepticus after sixty minutes includes high doses and continuous infusions of Pentobarbital or Tiopenthal, and more recently also Propofol, a potentially useful drug.%L'état de mal convulsif épileptique généralisé ou partiel demeure encore de nos jours de mauvais pronostic avec une mortalité chez la personne âgée de 13 à 35%. Il est impérieux de recourir à un plan de stratégie thérapeutique bien défini durant les 60 premières minutes. Le traitement d'urgence nécessite l'administration de substances à des doses efficaces par voie intraveineuse avec en première intention le clonazépam suivi des phénytoïnes, et si nécessaire après 60 minutes, une neuroanesthésie.
