INDUSTRIALISATION DES SOINS ET GESTION DE L'ALÉA : LE « TRAVAIL D'ARTICULATION »AU BLOC OPÉRATOIRE,DÉTERMINANTS ET OBSTACLES

À la lumière du concept d'agencement organisationnel, la recherche questionne les impacts sur le travail réel du tournant gestionnaire des établissements de soins provoqué par les nombreuses réformes du système de santé. Notre étude de cas réalisée au sein du bloc opératoire d'une clinique privée souligne le rôle primordial du travail d'articulation effectué par les personnels pour faire face à l'événement. La communication est placée au cœur de l'agencement qui se construit en situation perturbée. Notre étude montre que les dispositifs gestionnaires déployés dans ce contexte de rationalisation des soins peuvent constituer un soutien essentiel à ce travail d'articulation, finalement apparenté à un travail de régulation au travers duquel les acteurs s'approprient ces dispositifs. Toutefois, nous identifions au sein de l'agencement un certain nombre de contradictions qui pèsent sur ce travail. Nous en appelons alors à de nouvelles formes de rationalisation du cadre de l'action tournées vers le soutien de ce travail de régulation.%Using the concept of an organizational arrangement, this research inquires into the impact on work of the managerial turn made in medical establishments owing to the many reforms of the French health-care system. This case study of an operating room in a private clinic draws attention to the "work of articulation" whereby the staff coped with this new situation. Communication was placed at the center of the organizational arrangements worked out in a disruptive situation. The managerial procedures used in the context of rationalizing care provided the needed support for this work of articulation, which was ultimately similar to the work of regulation, whereby the persons involved appropriated these procedures. Within this arrangement however, certain contradictions are pointed out that had an impact on this work, whence a call for new forms of rationalization of the framework of action so as to support this work of regulation
