The crew of the El Faro was not doomed because they were bad mariners, bad people, or bad leaders. The crew was doomed because they were trapped in an industrial age playbook. They tried the best they could, calling the plays they knew and had been programmed to play. What they needed was a new playbook. The recovery of the black box is significant and sheds light on how teams could operate in ways that balance thinking, learning, and robust decision-making against goal-oriented "doing" behaviors. As leaders, we owe it to our teams to pause our busy schedules, control-the-clock for ourselves and our teams, to make sure we have time to do it right.
