A global adhesives manufacturer has been fined £173,332 for failures that resulted in a reversing lorry crushing a maintenance worker to death against a forklift truck. The incident occurred on 1 July 2010 in the transport yard of Mapei UK's Halesowen plant in the West Midlands. Andrew Davies was using a forklift to remove a waste receptacle containing powder from process machinery and then tip its contents into a one-tonne bag held in a cage. After lifting the container above the bag and cage, he was standing in front of the truck facing the raised forks. At the same time, a lorry driver, who had been asked to move his vehicle by another driver to allow access to the loading area, started reversing his 18-tonne rigid-back lorry. He ran into Mr Davies, who was in a blind spot, pushing him onto the truck's forks. Mr Davies died instantly from head injuries.
