Basic guarding failures result in worker severing arm

A manufacturer of concrete products has been fined more than a fifth (21.7%) of its most recent pre-tax profits for machinery failures that led to the severing of a worker's arm below the shoulder. The incident occurred on 18 May 2015 at Concrete Fabrications' plant in Bristol. The injured man, who wishes to remain anonymous, was repairing a misaligned conveyor belt on one of three processing lines where aggregate was bagged and sold. This required him to use a spanner to adjust tensioning rods inside the machine's guards, close to the conveyor belt and rotating tail pulley. As he used a hammer to remove aggregate that had built up on the tensioning rod, the hammer was dragged into the rotating tail pulley along with his arm.
