An Investigation Into the Root Cause of a Spill From Procuring and Handling of Lubricants in Intermediate Bulk Containers

Root cause analysis is a rigorous and expensive process that is not usually used to investigate relatively minor spills. However, given the ubiquity of portable, intermediate bulk container use throughout the world and the cumulative effects that minor spills can have on the environment, the use of such analysis on a minor spill-as this case describes-provides information of general application internationally and at work sites large and small. As the author explains, engaging in root cause analysis revealed an unexpected root cause that most likely would have been overlooked if a less rigorous investigation had been conducted.
