I was working at a semiconductor manufacturer, and we had just designed an evaluation platform that showed the excellent performance of our new state-of-the-art product. This platform included the board with a power switch and LED indicators in a case, a power brick, and the software to run the board. We had done thorough worst-case design analyses, reliability studies, power-cycling studies, and all the rest of what you normally do for new products. We found no problems, the design got the go-ahead, and we put it into production.
