Persistent truncus arteriosus (PTA) is an uncommon but complex congenital heart anomaly. Nowadays neonatal surgery is a common practice, and unrepaired cases are very rare in the current era. Typically, in the absence of surgical repair, patients do not survive beyond the first 6 months of life due to congestive heart failure related to large pulmonary blood flow, or rarely survived with severe pulmonary arterial hypertension. A 36-year-old woman with a known uncorrected PTA was referred to our tertiary centre for preconceptional diagnostic evaluation. Auscultation revealed a loud systolic murmur over the precordium. Oxygen saturations were 88% at rest.
