Cryosurgical Management of Symptomatic Vulvar Vestibular Papillomatosis

Vulvar vestibular papillomatosis (VVP) isan anatomic or functional variant ofthe normal genitalmucosacommonlymistaken for genitalwarts. Wereport thiscaseto highlight the need for dermato-venereologists to becomefamiliar with this rareentity, thusavoiding unnecessary concernsandinvestigations,and to considercryotherapy asatreatmentmodality in symptomaticcases.A21-year-old womanmarried for 1 year presented with smallraised lesions on her vulvaassociated with pruritus of 6-month duration. Sheandher partner werein a monogamous relationship withoutany history ofextramaritalsexualcontacts. Examination showed multiple, small, soft, shiny,smooth-surfaced, non-tender monomorphic papules 1–3mmin diameter with bases separatefromeach other,colored sameas that ofadjacentmucosa, symmetrically distributed over inneraspect oflabia minoraand vestibule with negativeacetowhitetest [Figure 1]. These werethetypicalclinicalfindings ofVVP. Dermatoscopy usingHeine NC2 Dermatoscope(Heine Optotechnik, Herrsching, Germany; ×10 magnification, polarizedmode) showed uniformly sized teardrop-shaped transparent juxtaposed papillae with separate bases with irregular vascular pattern in thecoreconfirming the diagnosis.
