A22-year-old boy presented to ourcenter with subtotalrhegmatogenous retinal detachment (RRD). His best-corrected visualacuity (BCVA)was 20/1200 in theaffected eye. Posterior segmentexamination revealed largesuperotemporal horse-shoetear withmultiplelattices [Figure 1]aand [Figure 1]b. The patient underwent vitreoretinalsurgerywith an encircling band and silicone oiltamponade using 3Dheads-up visualizationsystem. [Figure 1]cand [Figure 1]d Intraoperatively, brilliant blue Gdyeand PFCL (perflurocarbon liquid) wereinjected to peeltheinternallimitingmembrane. However, microscope-integrated opticalcoherencetomography (mi-OCT) revealed subretinalfluid persistentat the posteriorpolealongwithRPE elevation (doublelayer sign) under the PFCL [Figure 1]eand [Figure 1]f. Adiagnosis of underlying type 1 choroidalneovascularization (CNV) was madeand half-doseintravitreal bevacizumab (0.625 mg/0.025 ml)[1] was injected after silicone oil tamponade.
