A20-year-old male presented with a history ofwhitening of nails whichwas insidious in onset progressed to symmetricaltotalwhitening ofallfingernailsand partialwhitening ofalltoenails within 1 year [Figure 1]. Examination revealed opaque porcelain-white discoloration ofallfingernails with partialinvolvement ofalltoenails. Onblanching, white discoloration remained thesame. Asecond similarcase was seen in an 18-year-old male with a history of progressive whitening offingernailsandtoenails for the past 3 years [Figure 2]. On examination, total porcelainwhite discolorationwas seen in allfingernailsand toenailsexcept thethumb and great toenailswhere partialleukonychia was present. There was no history of preceding trauma, illness, drug intake, orexposureto chemicalsand no similarchanges wereseen inthefamilymembers of both cases. Therest ofthe mucocutaneousand systemicexaminationwas unremarkable. No skin lesions suggestive of psoriasis or lichen planuswereseen. Routine hematologicaland biochemical blood investigations were normal. Nailclipping Periodicacid Schiffstain (PAS)and 20%KOHtestshowed nofungalelementsand theculturerevealed no fungal growth. On the basis of history and clinicalexamination,a diagnosis ofidiopathicacquired truetotaland partialleukonychia was made. Both patients werecounseled regarding the benign nature ofthecondition,and there were no changes noticed in leukonychia during thefollowupperiod.
