Response to the Association Between Orthostatic Hypotension and Nocturnal Blood Pressure May Explain the Risk for Heart Failure

We appreciate the letter submitted by Grossman et al1 in response to our recent publication on the association of orthostatic hypotension (OH) with incident heart failure (HF) within the Atherosclerosis Risk in Communities cohort.2 Indeed, reverse diurnal blood pressure patterns, including nocturnal nondipping (decrease of nocturnal blood pressure ≤10% from daytime blood pressure) or reverse-dipping (increase of nocturnal blood pressure compared with daytime blood pressure) could explain the association between OH and HF, particularly in those with hypertension.The authors note that in their previous study of 185 patients referred for 24-hour ambulatory blood pressure monitoring either to confirm a hypertension diagnosis or to assess …
