Delayed diagnosis in a spinal epidural abscess is related to poor outcome. This case highlights an atypical presentation of a spinal epidural abscess. Early diagnosis leads to early therapy and better outcome.We present the case of a man who had a good clinical history and CSF findings to support a diagnosis of subarachnoid haemorrhage.Following a negative cerebral angiogram, a revised diagnosis of ?spinal? subarachnoid haemorrhage was made. Further imaging however revealed an epidural abscess.We use this to discuss the investigation of suspected ?spinal? subarachnoid haemorrhage. We also discuss the presentation of an epidural abscess and highlight that the clinical presentation may be atypical.
