This article presents a case report of successful laser treatment of posttraumatic iris cyst and centration of pupil in phakic eye. At the baseline best corrected visual acuity (BCVA) preoperatively was 0.3, the IOP – 18mmHg. The treatment consisted of two steps. At the first step a dissection of iris cyst was performed by the YAG laser (wavelength – 1064nm, spot diameter – 8μm, pulse duration – 4 nanosec.). At the second step a restoration of pupil centration was achieved by the YAG laser iridotomy with a previous coagulation of section site with the argon laser. After treatment the cyst was reduced, the pupil expanded upwards by2 mmopening the central optic zone with a significant BCVA improvement up to 0.6. The lens transparency did not change. Visual field and IOP were within a normal range. Application of modern laser techniques allows to achieve good functional outcomes with a minimum risk of anterior capsule injury in the phakic eye with a posttraumatic iris cyst and decentration of pupil.?.
