Patient safety has become a focus of clinical care and researchin recent years. However, the potential for medical careto cause harm has been appreciated throughout the historyof medicine [1]. The safety culture of an organization is theproduct of individual and group values, attitudes, perceptions,competencies, and patterns of behavior that determinethe commitment to, and the style and proficiency of, an organization’shealth and safety management. It is important forhealthcare organizations to establish a safety culture to ensurethat patients are not inadvertently harmed by errors in the carethat was supposed to heal them [2]. Main objectives of the crosssectionalstudy were to explore the attitude and perception ofretail pharmacists in patient safety, to identify the strength andareas for patient safety improvement and to examine thecurrent practice in patient safety in Malaysian retail pharmacysetup. Malaysian registered pharmacists, working in theretail pharmacies in the study site and willing to participatein this survey, were included.Those who are not willing to participateand did not meet the inclusion criteria for the surveywere excluded from the study. The study was carried out inSelangor, Malaysia. This Pharmacy Survey on Patient Safetyemphasizes patient and medication safety and qualityassuranceissues. The survey includes 36 items measuring 11composites. In addition to the composites, the pharmacy surveyincludes three items about the frequency of documenting differenttypes of mistakes, three items about respondentbackground characteristics, an overall rating question, and asection for open-ended comments. The survey has a total of43 items [3]. It is a validated questionnaire used in many countriesfor assessing the patient safety in pharmacies.
