Negligent failure to diagnose a fetal anomaly at the 20 week scan in September 2006; it was noted that three arm bones were present in respect of each fetal arm, and that no abnormality had been detected. The fact that only two of the three bones were present in both fetal arms was missed. Had this fetal abnormality been detected, the Claimant would have opted for a termination of pregnancy before reaching 24 weeks gestation.
