对社会管理的几点思考

This study was prompted by previous observations that apoE genotype has profound effects on vitamin K status. The sensitivity to anti vitamin K therapy with phenprocoumon, a coumarin derivative, was investigated retrospectively in 30 cardiological patients and related to their apolipoprotein E (apoE) genotype. Eight patients with the apoE variant E4 had significantly lower prothrombin content than the other 22 patients (26.4 vs. 32.3 % of normal, p < 0.02), but had used similar doses of phenprocoumon (14.1 vs. 14.9 mg/week). A statistically significant trend of prothrombin content in the order E2/3 > E3/3 > E3/4+E4/4 was detected by one-way analysis of variance. This is the first report of a common genetic polymorphism influencing sensitivity to anticoagulant treatment with anti vitamin K drugs.%Der Apolipoprotein-E-Genotyp hat einen deutlichen Einfluß auf den Vitamin-K-Status. Wir haben deshalb die Sensitivität einer Anti-Vitamin-K-The-rapie mit Phenprocumon, ein Cumarinderivat, retrospektiv bei 30 kardiologischen Patienten untersucht und auf ihren ApoE-Genotyp bezogen. 8 Patienten mit der ApoE-Variante E4 hatten eine signifikant niedrigere Pro-thrombin-Konzentration als die anderen 22 Patienten (26,4 vs. 32,3 % der Norm, p < 0,02), sie hatten jedoch eine ähnliche Dosis von Phenprocumon (14,1 vs. 14,9 mg/Woche) erhalten. Die Varianzanalyse ergab einen statistisch signifikanten Trend des Prothrombingehaltes in der Reihenfolge E2/3 > E3/3 > E3/4 + E4/4: Zum erstenmal wird in diesem Bericht aufzeigt, daß ein häufiger genetischer Polymorphismus die Wirksamkeit einer Antikoagulanzien-Behandlung mit Anti-Vitamin-K-Pharmaka beeinflußt.
