原发性高血压患者血尿酸水平与左室舒张功能关系的研究

         

摘要

目的:探讨原发性高血压患者血尿酸水平的变化及其与左室舒张功能的关系。方法选择原发性高血压患者62例,按是否有左室舒张功能不全分为两组,均检测血尿酸、甘油三酯、总胆固醇等水平。心脏超声检测评估二尖瓣舒张早期血流速度(E)与二尖瓣舒张晚期血流速度(A)的比值(E/A)及E值与组织多普勒瓣环速度(Ea)的比值(E/Ea)以评价左室舒张功能。结果两组患者性别、年龄、病程、体重指数、收缩压、舒张压、空腹血糖、肌酐、总胆固醇及甘油三酯比较差异均无统计学意义(均P>0.05);左室舒张功能不全组血尿酸水平明显高于左室舒张功能正常组(P<0.01)。左室舒张功能不全组患者血尿酸水平与E/A呈明显负相关(r=-0.779,P=0.000),与E/Ea呈明显正相关(r=0.611,P=0.002)。结论原发性高血压伴左室舒张功能不全患者血尿酸水平升高。高尿酸血症可能是原发性高血压患者左室舒张功能不全的一个标志。%Objective To investigate the correlation of serum uric acid levels with left ventricular diastolic function in pa-tients with essential hypertension. Methods Sixty two essential hypertension patients were divided into two groups: with or without left ventricular diastolic dysfunction.Serum uric acid,cholesterol (TC) and triglyceride (TG) were measured.Left ven-tricular diastolic function was assessed by pulsed Doppler tissue imaging and was determined by ratio E wave/A wave and E/Ea. Results There were no significant differences in sex, age, disease course, BMI, systolic blood pressure, diastolic blood pressure, fasting blood glucose, creatinine, total cholesterol and triglyceride between two groups (P>0.05). The serum uric acid levels were significantly higher in left ventricular diastolic dysfunction patients (P<0.01), which were negatively correlated with E/A (r=- 0.779, P=0.000) and positively correlated with E/Ea (r=0.661, P=0.002). Conclusion Serum uric acid levels are increased in essential hypertension patients with left ventricular diastolic dysfunction, indicating that hyperuricemia might be used as a marker of diastolic dysfunction in essential hypertension patients.
