中西医结合治疗脾肾两虚型胎动不安的疗效观察

摘要

Objective To observe the clinical efficacy of TCM prescription Antai-III acupoint application combined with progesterone injection and modification of Yulin decoction on the treatment of threatened abortion with deficiency of spleen and kidney. Methods A total of 46 cases of patients with threatened abortion with deficiency of spleen and kidney during April 2012 to September 2013 were selected as the control group, while the other 46 cases of patients during September 2014 to September 2015 were enrolled as the observation group. In control group,patients received progesterone injection and decoction,whereas patients in observation group were treated by prescription Antai-III acupoint application at Shenshu and Pishu. Both groups were treated for two treatment courses(14 days). The improvement of symptoms,including colporrhagia,lower abdominal pain,soreness of waist,thin sloppy stool,abdominal distension and anorexia between two groups before and after therapy were observed and compared as well as the change of total TCM syndrome integral. Results In observation group,the total effective rate and cure rate were respectively 84.78% and 47.82%,while 78.26% and 39.13% in control group. Total TCM syndrome integral was significantly decreased after treatment. There was significant difference (P<0.05) between the two groups. Furthermore,the improvement of symptoms,such as soreness of waist and lower abdominal pain,were obviously better in the observation group than that of control group(P<0.05). Conclusion Prescription Antai -III acupoint application combined with progesterone injection and decoction can effectively improve the therapeutic effect of threatened abortion with deficiency of spleen and kidney.%目的：观察安胎3号方穴位贴敷联合黄体酮针及中药温土毓麟汤加减治疗脾肾两虚型胎动不安的临床疗效。方法选择2012年4月至2013年9月脾肾两虚型胎动不安患者46例为对照组，2014年9月至2015年9月脾肾两虚型胎动不安患者46例为观察组。对照组给予黄体酮针及中药口服，观察组同时予安胎3号方穴位贴敷肾俞、脾俞，两组均以7d为1个疗程，共2个疗程。分别观察两组治疗前后阴道出血、下腹坠痛、腰酸、大便稀溏、腹胀纳差等症状改善情况，总的中医证候积分下降情况。结果观察组总有效率84.78%、治愈率47.82%；对照组总有效率78.26%、治愈率39.13%，两组治疗后中医证候积分均较治疗前明显下降（P<0.05），观察组腰酸、下腹痛症状改善情况优于对照组（P<0.05）。结论安胎3号方穴位贴敷联合黄体酮及中药口服治疗脾肾两虚型胎动不安可明显提高疗效。
