浙江丽水地区非结核分枝杆菌菌种鉴定及耐药性分析

         

摘要

目的 探讨丽水地区非结核分枝杆菌(NTM)菌种分布特点及其耐药情况,为临床诊疗提供参考.方法 收集2013年1月至2015年12月丽水地区分枝杆菌培养阳性的菌株,用PNB/TCH生长试验初步鉴定菌种,利用基因芯片对分离的NTM作进一步菌种鉴定;对鉴定为NTM的菌株采用比例法进行6种药物敏感性试验.结果 共分离鉴定分枝杆菌属671株,其中NTM 81株(12.07%).NTM共检出9个菌种,以胞内分枝杆菌、鸟分枝杆菌和龟-脓分枝杆菌为主,占比分别为59.26%、16.05%和9.88%.NTM耐药率总体较高,特别是龟-脓分枝杆菌和戈登分枝杆菌对6种药物全部耐药.结论 NTM的耐药率高,根据药敏制定合理的治疗方案是治疗成功的关键.%Objective To investigate the distribution and antibiotic resistance of nontuberculous mycobacterial(NTM)in Lishui area,so as to provide essential information regarding the selection of antibiotic therapy. Methods Mycobacterium culture positive strains were collected from January 2013 to December 2015 in Lishui area,NTM was identified by PNB/TCH culture method,and gene chip was used on the isolated NTM strains for further identification.Identified as NTM strains proportional method for the six drug sensitive test. Results A total of 671 strains of mycobacterium were isolated and identified,including 81 strains of NTM(12.07%). There were 9 species of NTM detected,the main NTM among which were the mycobacterium intracellular(59.26%),mycobacterium avium(16.05%)and turtle or mycobacterium abscessus(9.88%). The drug resistance rate was higher,especially the turtle or mycobacterium and Gordon mycobacterium which reached 100.00% to 6 kinds of drugs. Conclusion The drug resistance rate of NTM is high,it is critical to develop the treatment programs in accordance with the result according to the drug susceptibility testing.
