小针刀为主治疗第三腰椎横突综合征疗效观察

摘要

目的 观察小针刀为主治疗第三腰椎横突综合征的疗效.方法 将286例第三腰椎横突综合征患者随机分为治疗组148例和对照组138例.治疗组采用小针刀分解治疗,辅以局部神经阻滞和腰背肌功能锻炼;对照组采用针刺配合推拿及腰背肌功能锻炼治疗.两组患者治疗前后均采用视觉模拟评分(VAS)法评定疼痛程度,并测量腰屈曲活动度.结果 两组患者VAS评分及腰屈曲活动度与同组治疗前比较,差异均具有统计学意义(P<0.01,P<0.05).治疗组治疗后VAS评分及腰屈曲活动度与对照组比较,差异均具有统计学意义(P<0.01).两组患者随访6～24个月,治疗组VAS评分与腰屈曲活动度均优于对照组(P<0.01).结论 小针刀为主治疗第三腰椎横突综合征可明显改善患者疼痛及腰屈曲活动度,且远期疗效稳定.%Objective To investigate the efficacy of knife needle therapy as main treatment for transverse process syndrome of third lumbar vertebra.Methods Two hundred and eighty-six patients with transverse process syndrome of third lumbar vertebra were randomly allocated to a treatment group of 148 cases and a control group of 138 cases.The treatment group received knife needle lysis aided by local nerve block and lumbodorsal muscle functional training and the control group, acupuncture plus massotherapy and lumbodorsal muscle functional training.Pain severity was assessed using the Visual Analog Scale (VAS) and lumbar flexion was measured in both groups before and after treatment.Results There were statistically significant pre-/post-treatment differences in the VAS score and lumbar flexion in both groups (P＜0.01,P＜0.05).There were statistically significant post-treatment differences in the VAS score and lumbar flexion between the treatment and control groups (P＜0.01).The VAS score and lumbar flexion improved more in the treatment group than in the control group at 6-24 months' follow-up (P ＜ 0.01 ).Conclusion Knife needle therapy as main treatment can markedly improve the pain and lumbar flexion in patients with transverse process syndrome of third lumbar vertebra and has a stable long-term therapeutic effect.
