中西医结合疗法对老年桡骨远端骨折愈合质量及术后遗留腕关节僵硬的防治研究

摘要

Objective:To observe the efficacy of integrated traditional Chinese and western medicine(TCM and WM)in the treatment of distal radius fracture healing and left wrist stiffness in elderly patients.Methods:90 elderly patients with distal radius fractures treated in our hospital were divided into the observation group and the control group by random number table,with 45 cases in each group.The control group was treated with open reduc-tion and internal fixation of fracture,and were given manual therapy and functional exercise.The observation group was treated with combination therapy of traditional Chinese medicine and western medicine on the basis of the control group(self made bone activating blood circulation decoction and Chinese medicine external washing).The clinical symptoms,fracture healing and wrist function of the two groups were compared.Results:The scores of abnormal appearance,skin temperature increase,joint pain and joint swelling in the observation group were significantly lower than those in the control group(P < 0.05).14 days and 30 days after operation,the RUSS scores in the observation group[(3.91 ± 0.29),(6.15 ± 0.35)]were significantly lower than those in the control group(P < 0.05),and the fracture healing time[(50.16 ± 4.02)d]was significantly shorter than that in the control group(P < 0.05).At 2 months after operation,the excellent and good rate of wrist function in the observation group(91.11%)was signifi-cantly higher than that in the control group(73.33%)(P < 0.05).Conclusion:Internal and external application of traditional Chinese medicine can significantly improve the healing quality,shorten the healing time and significantly improve the wrist function in elderly patients with distal radius fractures to avoid the occurrence of postoperative w rist stiffness.%目的:观察中西医结合疗法对老年桡骨远端骨折愈合质量及术后遗留腕关节僵硬的防治效果.方法:采用随机数字表法将我院收治的老年桡骨远端骨折患者90例分为两组,各45例.对照组行骨折切开复位内固定术、并给予手法治疗及功能锻炼,观察组在对照组基础上自拟接骨活血汤和中药外洗.对比两组术后临床症状、骨折愈合情况及腕关节功能.结果:观察组外观畸形、皮温升高、关节疼痛、关节肿胀评分均低于对照组(P<0.05).观察组术后14 d、30 d RU SS评分为(3.91 ± 0.29)分、(6.15 ± 0.35)分明显低于对照组(P<0.05),骨折愈合时间(50.16 ± 4.02)d显著较对照组短(P<0.05).术后2个月观察组腕关节功能优良率为91.11% 显著高于对照组73. 33%(P<0.05).结论:中西医结合疗法可显著提高老年桡骨远端骨折愈合质量、缩短愈合时间,并可显著改善患者腕关节功能,避免术后腕关节僵硬的发生.
